Client
After 48 years of dedicated service, Solve-TAD, a charity organisation that creates innovative and custom assistive technology solutions for people living with disabilities, has rebranded as Freedom Solutions Australia. (Source)
Task
To redesign Solve-TAD’s client referral form by updating it with a more dynamic and responsive UI (e.g. making it more aesthetically-appealing and adding personalised accessibility features) and adapting the format (e.g. removing unnecessary questions and changing open-ended questions for greater control) in order to make it more engaging, concise and user-friendly.

The Current Referral Form
Solve-TAD's main business problem is its client referral forms, which are a crucial part of its service delivery process. Customers are taking a considerable amount of time to fill out these forms, with a significant percentage of potential customers leaving them incomplete or submitting them with important information missing.
How can Solve-TAD generate more client leads and referrals to increase their customer acquisition and conversion rates?

Our team received high appraisals from Solve-TAD's Promotions Coordinator, Meadhbh Roche. As of now, Solve-TAD’s current referral form has an improved user experience inspired by our prototype, incorporating many of the proposed features.
